File a Notice of Change For Your Corporation

This form is used for existing Ontario or Federal corporations (named or numbered) that will be making the following changes to the corporation.

• Corporate Address
• Directors Address
• Changes to the existing directors either adding/removing or changing position.

• If you wish to change the corporate structure (i.e. corporation name change or changing the share structure, you need to do a Corporation Amendment instead).
